Changes to registration during Covid-19 Pandemic

Due to the current COVID-19 pandemic, patients are asked to register without coming into the practice. To register you will need to complete our online registration form

 

New Patient Registration

We can only accept new patients who live in our practice area. 

You can register by completing our secure online registration form. Please click here. 

Please complete all parts the form in full including your NHS number which can be obtained from your previous GP. 

Sumbit the form online to the surgery with two forms of identification, one of which must contain proof of your address. A driving licence or utility bill would be ideal. These will be photocopied by reception.

We have equipment in our waiting room for you to measure your blood pressure. Please enter the information given on the printout onto the form.

All New patients can request an optional health check with the Practice Nurse, please enquire when registering.
